Medication Safety Pharmacist

Evaluate medication-use decisions, interaction and contraindication reasoning, formulary considerations, and patient-safety analysis in de-identified cases.

Review whether AI-generated recommendations account for indication, dosing, organ function, allergies, monitoring, duplication, and clinically significant interactions.

Identify unsafe therapy choices, missing patient factors, unsupported substitutions, and situations that require prescriber clarification or urgent escalation.

Provide concise corrections grounded in evidence-based pharmacotherapy and the practical realities of medication-use systems.

Remote, project-based contract work. Scope and hours depend on project matching; the listed hourly range applies to matched work.

Requirements

• PharmD with active or recently active pharmacist licensure
• 7+ years in hospital, ambulatory, managed-care, or medication-safety practice
• Board certification such as BCPS, BCACP, or equivalent specialty experience preferred

$130–$195/hr
